The Kind Of Resident This Serves Best.
Residents nearing the end of life who want to remain in a familiar home, surrounded by familiar caregivers, rather than transitioning to a hospital or hospice facility.
How We Actually Do This.
When a resident enters hospice, we don't transfer them out — we partner with their hospice team and continue providing the daily care they're used to. Our caregivers know the resident; the hospice nurse knows their medical course. Together we make sure the resident's final weeks are as peaceful, comfortable, and dignified as possible. Family is welcome at any hour.

"Mom passed on a Tuesday morning at Simon, with her family around her and Genet holding her hand. We didn't have to move her to a hospital. We didn't have to say goodbye in a sterile room. She got to leave from her home. That's a gift."
